I disagree on this. Depending on context, I would write out the full
form + abbreviation in brackets, regardless of whether it's a holdover
from print or not...it makes it immediately usable by all users.

As for the remaining occurrences, again: depends on context, but I'd
try to mark all occurrences up with ABBR.
