Not all content tagged when converting from Word or PPT to PDF

For the past few months, I've been noticing that sometimes, not all content
is tagged in a PDF file when I convert it from either Word or PowerPoint. I
don't recall this being an issue before, but it definitely is now. I can
understand why this might happen with a PowerPoint presentation, but not a
Word document.
Has anyone else had this problem? What can be done to fix it?
