According to Doug Wakefield, formerly from the Access Board and what SSA
has adopted, "anytime a webpage requests information from the user, the
user is interacting with a form. So, just entering a user name and
password is still a two line form at its minimum". Additionally, Doug
said that to be accessible a form must permit a user to read any
instructions, complete and submit the information.
