So, Rockie, based on my experience as a content manager, I would make
this form myself in google forms, and I would include blanks for "alt
text (describe the image)" and whatever other accessibility data you
need. I feel like alt text is the main thing that you would need
people who provide content to provide, the rest of the accessibility
stuff is the responsibility of developers etc.
