Greetings from a Mac zealot. You can use pdf pin from Smile Soft wear to fill out pdf forms with voice over.

It makes forms pretty nicely accessible, although other pdf apps will give you a better reading experience. Thats how I do my taxes. Preview is terrible by all accounts and Apple really needs to step up its game. I feel like pdf's are the last great hurtle in Apple's accessibility.
By the way there is a free app called Skim which gives you access to headings, which is another shortcoming of Preview.
