Hello Jan,

I tried to use this form with JAWS 7.0 and found the following issues:
1. Alternate text for an image was missing. In the absence of the alt, JAWS
read 'Graphic' living the users guessing what the image might be.
2. Headings were not marked up.
3. Labels for the form fields were assigned an incorrect reading order, I.e.
For the 'Name' fields, labels were read after the form fields
by JAWS. Same was the case with 'Address' fields.
4. Signature fields and Date fields were read as text assuming they have to
be filled up after taking printout .
5. Instruction "* required..." was placed at the bottom of the page which is
likely to go unnoticed by many of the users.
6. "Print" button was read as the first item in the tab order.
