Hi -
As discussed, you can't make an accessible PDF from MS Word for Mac 2011.
No formatting is retained when you convert it.
Libre Office which is Mac compatible does allow you to make accessible PDFs
on a Mac. It retains alt text and formatting markup.

P.S. I would never recommend saving a MS word doc as HTML. There are
probably many bigger markup issues than just null alt text.
