PDF table editor disrupting reading order?

When using the Adobe Acrobat X table editor to merge or split cells, the newly defined cell moves to the wrong place in the reading order. The newly defined cell appears outside of the hierarchy of the table.
I am blind and my coworker is sighted. We're trying to learn about making our PDFs accessible, but this has got us stumped.
What are we doing wrong?
