Linearized tables and more

Hi folks,
I'm looking for a nice, neat example of a linearized layout table to
illustrate the concept. Does anyone have an example I could adapt?
And, while I'm here, I've created this example for grouping documents:

The text-based browser Lynx will build a small menu of links at the top of
the page that reads like this:
# Contents Question 12 Question 14 Glossary Index
Question: Have I included too many documents?
Question: Is there a way to include separating characters?
