Making Text Editors?

Hello,
What is the difference between the editor used in code Pen or JS Fiddle and
the editor used in Monaco? The first is not accessible to a screen reader
and the second is. I know it has something to do with showing text as an
HTML element rather than in an editable way, but I don't know how to
describe it.
I just heard about the Iodide project:
https://alpha.iodide.io/tryit/
which is pretty much Google docs for coders and researchers, and they are
using the first method of editing and they need to use the second method. I
opened an issue:
https://github.com/iodide-project/iodide/issues/1749
But I want to change the title to be more technical and pointed.
Does anyone know the difference between these two editor modes?
