Mark,

You can use a span with aria-hidden="true" that holds the leaders, which would fall within the parent span. Example:

Chapter 1
<span aria-hidden="true">
------------------------------
</span>
Page 5
</span>
Screen readers would only read "Chapter 1, Page 5"
