I just used the following process to add the PDF/UA Identifier to a document:

1. In Acrobat Pro click File > Properties to open the Document Properties dialog.
2. Click the Description tab.
3. Click the Additional Metadata button.
4. Click the “Advanced” option near the top left of the dialog.
5. Click the Append button. A dialog will open, prompting you to select a file.
6. Locate and select the pdfUA-ID.xmp file and click the Open button. If you don't have the file, you can download it from https://taggedpdf.com/xmp/pdfUA-ID.xmp.
7. Click all the OK buttons.

This added the following 3 items that were not there previously:

1. http://www.aiim.org/pdfua/ns/id
2. Dublin Core Properties (dc, http://purl.org.dc/elements/1.1/)
3. http://www.aiim.org/pdfa/ns/extension/

PAC3 verified that the PDF/UA Identifier was set.

I then deleted http://www.aiim.org/pdfua/ns/id. PAC3 verified that the PDF/UA Identifier was no longer set.

I then deleted http://www.aiim.org/pdfa/ns/extension/, which made no different that I can tell. I was not able to delete the Dublin Core Properties, but again this didn't seem to make any difference.

Hopefully, that should give you enough information to fix your document.
