Yes. This.

If PDF software combines two tagged PDF files it is that software’s responsibility to account for the two sets of MCIDs being combined in order to avoid collisions. Very likely in this case, the software that was used for the merge doesn’t support tagged PDF at all.

The only solution here is to identify the responsible software and let its developer know that you care.

You are entirely correct that ISO 32000’s intended audience is PDF software developers.
