Blank table cells

Good afternoon, everyone -
I'm working on an evaluation form that's handed out to participants in a co=
urse. It's a Word file, with tables of areas of required actions and if it =
was "observed" or "not observed" as blank cells, meant to annotate a respon=
se.
I've made the recommendation for the end product to be a fillable PDF with =
checkboxes in those table cells, but I'm just curious everyone's stance on =
blank table cells in documents? Can anyone provide any direct references fo=
r the compliance standards of blank table cells?
