Online survey tools/software

I'm looking for a reasonably standards-compliant, reasonably accessible
survey generator for the web.

It can either be a hosted solution (like surveymonkey.com) or a
downloadable/installable solution, and all the better if I can tweak the
markup. For my specific needs, if it was an installable option it would
have to run on a Linux/Apache web host using PHP/Perl/Python
and MySQL/PostgrSQL.

Does anyone have any experience/recommendations?
