I manage a lot of website updates on behalf of other people, and I'm wondering if anyone has or knows of a form (preferably in MSWord, in layman's terms) to make sure I am receiving accessibility information as well as display information when people ask for website content changes.

I build / update HTML pages. People give me what they want as word files. I need to update my change request template to include fields for alt text, long descriptions, hidden extended text for "learn more" links, that kind of thing. But the subject matter experts for the content are non-technical so i need to explain this in plain language Does anyone have a good example of this?

Sounds like you're looking for a short and sweet checklist for the content owner to use in reviewing and submitting their files to you? would the MS Word checklist, and similar, at the HHS site (http://www.hhs.gov/web/508/checklists/index.html) fit your purpose? Probably much shorter, but something that to that effect, which documents that they were prompted to consider accessibility and provide you with things like appropriate alt-text, formatting was done with Styles, etc.?
