Dear all:

following some comments from Jukka, I've amended the accessible table
builder tool slightly:

* Scope or ID/Headers is now a switchable option
(before only used headers and id attributes)
* Empty attributes are not displayed in generated code
(before, if you cleared the border attribute, it
would still generate border="")
* The Caption now sits in front of the thead
(Oops, before it was within <thead> !)

To answer your question, I realise that this tool has a lot of options, and
that it may be difficult to understand in a screen reader. I'll be honest -
I have not tested the tool itself using a screen reader. Let me defend
myself though ;-)

* It's really a tool for web developers who are authoring for the
vis-impaired, not for vis-impaired users themselves
* I have not used tables much in this - the one that is generated
dynamically there is only there so that you type in the additional
parameters required
* Finally, the tool is totally reliant on scripting being enabled - there's
no <noscript> alternative to this one, so I admitted defeat on making the
tool accessible early on!

Anyway, I hope you people find the changes useful.

As ever, any further suggestions are welcome (I have another in the pipeline
- - to allow row headings to be used also, but that needs quite a bit more
work!)
