I have wondered about the same thing, but wasn't able to articulate
the question so clearly as you did, David. My own practice has been
to try to make the question immediately precede the radio button
choices as much as possible. But I have to think that explicit
linking of some sort would be a better and more reliable solution,
long-term.

<fieldset> can be used to enclose the radio buttons and the question would
be the <legend> for that fieldset.
