How does one use the keyboard to right-click and download a file? What
other options are there to allow someone to download a file and NOT open it
in a browser, PDFs for example.

Depends on the operating system and browser. It's simply option +
return on Macintosh/IE.

In Windows, shift + F10 opens the right-click menu, then "A"
activates "save target as". (Someone else may know a simpler way. I'm
not a Windows user.)

If a physical disability prevents someone from holding down two keys
at once, you have to enable accessibility features such as sticky
keys on the computer.
