Our online courses use a lot of narrated power points, where the slides automatically advance while an audio lecture plays at the same time. We want to provide alternative formats for students who cannot hear the audio and for students who can not see the slides.
Students need a way to match up their alternative format content with the content they are able to access from the narrated PowerPoint. The lecture content and slide content must still be synchronized.

Please tell me strategies you have used in similar scenarios. I have some ideas, but I'm very interested to learn what others have already learned about this.

can you make the powerpoint downloadable with the narration transcript and
extended image descriptions in the "notes" pane of the powerpoint? This
assumes that the powerpoint itself is accessible using standard
(accessible) templates with accessible content.

I have seen two different systems that do this type of thing. Both products
output were not accessible, meaning the various controls created were not
labeled. They ended up making a movie with Camtasia or Adobe Captivate to
integrate the slides, voice over and captions.

I use and recommend Techsmith's Camtasia: http://www.techsmith.com/tutorial-camtasia-captions-prior.html. It can generate synchronized close-captions from imported PowerPoint slides. It's a powerful tool and worth it's price in gold (so-to-speak). Check out some of its other features: http://www.techsmith.com/camtasia-features.html. It's also very user friendly. With little training you can get up and running very quickly and produce quality products in various formats.

Microsoft Research used to post invited talks with a three window
synchronized presentation: the video, the slides, a transcript. The current
line in the transcript was highlighted to match the audio/video.

Unfortunately, either what I remember was an isolated event, or they have
changed their interface, as they now have uncaptioned video and a list of
component files to download.

Personally, I loved the ability to skim through a long transcript to find
the section I was interested in and play from there by just hitting enter
with the cursor on the line of text.

+1

I would love it so much, if all the videos had transcriptions next to them, and I could skim through the transcription, and jump right to the portion of the video or presentation that I am really interested in.
