Subject: Accessible Survey provider?

A research colleague asked me for a recommendation for a vendor who can produce a survey using a survey tool that is accessible.
So, by "vendor," we mean an external consultancy or company that would put the survey content together and (I believe) also host it. The survey tool they use would have to meet WCAG 2.0 level AA.
