I've been asked to make a recommendation for a fax cover sheet that is downloadable from a site where customers apply for a product. This is an alternative to being able to upload required documentation electronically.

The cover sheet would have fillable form fields.

My gut is to recommend a well formatted Word document (RTF?), but I wasn't sure if there would be version compatibility issues. Is a correctly tagged PDF just as useful, or even more reliable?

IMO -- If the form is fillable PDF or HTML with a print CSS media type would be much more accessible than Word fillable forms.

Hi Judith,

A neat solution might be to allow users to fill in an HTML form online
and to download the filled form as a Word or PDF document. IMHO it can
be more difficult to make a Word or PDF form accessible, and some
users may not know how to fill it in.
