A post on Marco's Accessibility blog suggests that this may be possible with VoiceOver when running Snow Leopard. The post is about IoS4 but he mentions VoiceOver and Snow Leopard about 2/3 of the way down the page. I will need to wait till I get home to test - Windows world here.
