It lets you quickly generate accessible form elements by apppending unique
IDs to fields and tying them up with labels.

If you like it, please let me know (and feel free to add to favourites!)

FYI, I am working on adding to the tool to allow multiple form fields, such
as having 3x fields for telephone (but where you may only want one visible
element).

I will also be adding option to output as HTML 4 or XHTML very soon.
