It gets even weirder: adding a caption suppressed the summary in NVDA and Firefox. I think I'm just going to have to go with a "good enough" implementation that adheres to the best practices I can find, and hope for the best. There doesn't seem to be a way to make this perfect for everybody without adding a fair amount of brittle code to it.
