If you are thinking of creating a survey system from scratch, you might want to consider creating a fork of the free open source LimeSurvey CE product. They offer paid subscriptions for support but you don't have to buy them.

On the Features page they claim it is W3C compliant and screen reader compatible. However, I am extremely doubtful that this is actually the case, and they do not provide any information to support the claim. However, fixing the user interface issues has got to be a lot easier than creating an entirely new product.
I would definitely be interested in participating in such a project because we have been considering using LimeSurvey for several years.
