The youtube example would fail because it uses simple (unmodified) keys as
shortcuts and the keys work no matter where you are on the page. If I tab
to the comment area or the "up next" section in youtube and press K, the
video plays or pauses. If the K key only worked when the video itself had
focus, then it would pass because that's "Active only on focus". Since
youtube doesn't have a way to turn off the keyboard shortcuts nor a way to
remap the letters, it fails.

Gmail (web interface), on the other hand, has a settings for turning on/off
shortcut keys so it passes.
