I just tried this on my Mac, Catalina 10.15.3
I didn't have any languages installed prior to trying this. I went to the
"VoiceOver Utility" dialog and then the Speech settings. All that was
displayed was English so I added two new languages, Spanish and French, and
kept the default voices for those languages.
I then tried a simple test with
<button>english</button>
<button>hola</button>
<button>merci</button>
<button lang="es">hola</button>
<button lang="fr">merci>/button>
The first three buttons were announced in English.
The fourth button automatically switched to Spanish.
The fifth button automatically switched to French.
I didn't do anything special. It didn't matter if I tabbed to the buttons
or swiped left/right or used the left/right arrow keys. The language
switched automatically.
