Most of the elements you described have limited or no screen reader supprt:

* Description lists are at best treated like nested lists by most
screen readers, better to just use nested list, especially if the term
is marked up as a heading inside the list element. This can be
technically done entirely in Markdown
* Screen readers do noting with <em> and <strong> out of the box. ARIA
1.3 is adding roles which possibly maybe can help improve screen
reader support, or you can use CSS:content for screen readers to add
phrases like "start or emphasized text" etc. (see a detailed
description of the technique at
https://adrianroselli.com/2019/02/f87-css-generated-content-and-wcag-conformance.html).
For code, just use the markdown ``` to mark start and end of code.
Also remember you can write html elements right into Markdown.
