There are a number of possibilities, one of which is that this is a redundant image on the pasteboard i.e. outside the area of the document. The logo you can see might be artifacted, but the one you can't see isn't. We mostly see this sort of thing with documents authored in InDesign.

Are you sure Adobe Reader (or Acrobat) isn't in Reflow mode? If it is, it will be reading the Content panel, not the Tags panel. Or are you using some other PDF reader, such as a browser?

I am reviewing a PDF that has a logo at the top that does not show up in the "reading order" view but is pronounced by JAWS. Any idea why this is occurring? (I also can't find the image in the tag tree)
