Hi Rob,

Survey Monkey produces terrible code, they know it, and they don't care.

Try SurveyGizmo: http://www.surveygizmo.com. We've had very good
experience with them. They do have one or two widgets that cannot be
made accessible, but standard types of survey questions come out
quite well. This is a company that's committed to producing an
accessible end product, and if you find something that doesn't work
right, they're very open to hearing that and will work to fix it.
