tables without table headers

This probably sounds like a stupid question, but I've got a real-life reason for needing to know. How do screen readers users know if a table has been used for layout versus data if the developer did not use table header tags? Even if a table holds real data, but the developer(or the CMS) fails to use table header tags, does the screen reader user have to evaluate the content to determine if the table was used for layout versus data (or even pseudo-data)?
