Last time I knew, survey monkey doesn't associate question and answer,
e.g. use the question in the answers checkboxes as it should. This is
one real issue with survey tools--e.g. you should hear the question and
the checkbox or radio button answer together, or at the least associate
them with table linkage or other linkage/association. it is possible to
navigate back and forth with the screen reader, but the whole point of
associating sufficient information for such fields is to make them
usable.

Surveymonkey should work. I use it to complete surveys with JAWS and
thern't
any problems eusuallywith this site.
