Hi Birkir, VoiceOver reads the image with no alt text, it reads the filename. VO also reads the image with a space in the alt, reads "space image". It skips over the image with alt="" (null) and it is not navigable when switching to Images in the web rotor. There is a setting on iOS and OS X to navigate images: Always, With descriptions, or Never.
