empty headings perceived?

Hello,
I have noticed that our page editors (who use Contribute) often leave empty heading tags as they add/delete new things to a page. They have no idea they are doing this of course. Are these perceived by screen readers so as to be a pain in the rear?
The same goes for <p> </p> to create empty space on the page. It is my understanding these are not perceived.
If empty headings are perceived, but paragraphs are not, why is that?
