Does anyone know of a live demo/example of an accessible form with certain
elements that display ONLY when a specific option is selected?

For example:

Place of Birth: [US] vs. [Foreign Country]

...where US and Foreign Country selections would trigger their OWN drop
down lists for choosing the specific locations.
