The best options to my knowledge are:
- Microsoft Word 2010 or newer, plus CommonLook Office ( http://www.commonlook.com/CommonLook-office )
- Adobe InDesign CS 5.5 or newer, plus axaio MadeToTag plug-in ( http://www.axaio.com/doku.php/en:products:madetotag ).
Both Word and InDesign can export directly to tagged PDF, but that tagged PDF is often not sufficiently accessible. The above mentioned additional tools help to get to fully accessible PDFs.
The quality of tagged PDF can be checked with the free PDF Accessibility Checker (additional semantical / manual checking necessary!) - cf. http://www.access-for-all.ch/en/pdf-lab/pdf-accessibility-checker-pac.html .
