The process for creating accessible PDFs can either be very easy or stubbornly difficult.

PDFs made from Word documents are very easy if you use Word's built-in Accessibility Checker, (this is Windows only), first it should pass Adobe Acrobat's checker easily.

PDFs made from InDesign are less cooperative as InDesign seems to leave behind a lot of untagged elements. It took a lot of hunting to resolve these issues.
