I might be missing the point of the question, but the bigger picture of accessible design is that there is such a wide range of potential special needs and potential severities. With that array of issues, there is an even wider arrary of the ways in which people compensate. Ultimately UA detection would be fruitless, IMO.
What about the color blind? What about those with Parkinsons? What about people who are not totally blind but are legally blind?

Your best bet would be to simply build a completely accessible site that doesn't rely on alternative anything.
