In the past, I used PVII navigation for drop down fly out navigation
systems:

I know that it was very accessible (they really put a lot of time into
making it work with screen readers and keyboards) about 5 years ago,
haven't tested it in a while.

They don't make a Drupal module (which is a pain!), but in the past I have
put it into my Drupal template.
