Accessible survey assist request

Hi, does anyone have a good recommendation on a free method (plugin, code, etc.) to build a robust multi-page survey in Wordpress that is accessible to the survey taker? Ideally, the solution can also help organize the raw data into graphs and summaries. Or is that combination too much to ask for?

Any suggestions are appreciated!
