The Acrobat errors you list can arise for various reasons, and there is
not always one method to fix each specific error. I would start with the
form field and "annotation" issues. Sometimes fixing those might make
other errors disappear.

2) "Tagged form fields-failed"
Here I would guess that you have added Form tags, but you haven't marked
the "annotations" as Form fields. Acrobat doesn't automatically mark
form fields correctly when you simply use the "Form" button the "Touch
Up Reading Order" tool. Form fields that are not recognized
automatically often require additional manual tweaking. Maybe you can
try to follow the instructions provided by WebAIM to tag these unmarked
annotations?:
