Hi Itzel,
I recently had these sorts of problems, I found it useful to try to locate
the items in the Content panel, rather than the Tags panel in Acrobat, and
see if you can see if an existing image with an alt text was accidentally
split in two when exporting the doc, it sometimes results in one image with
alt text and one image without (or, in my case, a 1px wide sliver of an
image on the next page) and if it's a false image, you can delete the
element from the page in the Content panel, see if that helps your doc pass.
Was this doc created in inDesign? I know when creating links in a TOC,
there is a part of the link options (where you tell it to go to a page
instead of an external web link) where you set the alt text manually to
match the highlighted text that makes the link, which may seem redundant to
set alt text on text but may fix your problem, plus it helps screen readers
parse the text correctly instead of it just reading as "blank".
