need help with a table

Hi folks,

I was told that someone here would be able to help me. I have
attached two documents and I hope that I don't mess up the
archives. I am having trouble with a table and making sure
that it is fully accessible and makes sense to everyone else.
I realize that if it is accessible, it will make sense to
everyone. The html document is what I have done with it so
far. The Word document is what it is based on. Part of the
problem is that there are rows with three items that go
together throughout the row. I'm not sure if that makes sense.
Hopefully someone has some ideas. Thank you all in advance.
