I've been looking into a free and open source tool called "HUGR" which
facilitates the report generation process based on WCAG 2.1 AA or AAA (and
a few others). The tool itself is supposed to be very accessible: HUGR
Accessibility Testing App <https://hugr.app/>

You can track pass/fails with notes on each WCAG 2.1 guideline (1.1 Text
Alternatives, 1.2 Time-based Media, etc.) in a web-based spreadsheet-like
interface, then generate a report based on the data that you entered. It's
pretty quick to set up, so it might be worth looking at even just to
generate ideas.

I am still assessing the tool so I can't vouch for it from start to finish,
but it is promising so far.
