Subject: Form tags and specs in PDF's

Hello all,
I'm looking for some guidance remediating PDF forms. I'm using Acrobat Pro
and I thought I was doing a pretty good job, but I recently tried
CommonLook PDF Validator and I'm seeing a lot of failures and warnings,
some of which I don't understand. For example, I'm getting a lot of
warnings described as "no label text associated with this form field" but
all of the identified form fields are named and have tooltips. (They also
pass the Acrobat Accessibility Checker.)
Am I missing something? I try to put the form tags right after the
associated <P> tag. Is that incorrect?
Here's an image of my tags panel:

I'm also getting a failure for: "2 radio buttons share the same variable
name --- and hence would have the same tooltip (TU key)" Aren't radio
buttons in a group supposed to have the same name and tooltip, just
different radio button choices (under Options)?
Thanks in advance for any help you can provide.
