Does anyone have an idea why this form, created in MS Word 2016, doesn't
work correctly with Jaws?

It works find visually, in other words, checking the box works. But, when
Jaws is on, each category appears to have a checkbox and when one checks a
specific box, such as box number 2, it checks box 3.

Any advice would be appreciated.
