Subject: Tab Order and Screen Reader Synchronization Question

Hi All,
I notice that on some sites, as I read with my screen reader, and I run into something that would normally take focus, such as a link or button, that I can tab away from it and right back onto it, because the tab order stays in sync with the screen reader. On other sites, the screen reader finds the control or link, but the tab order is somewhere else. Perhaps this is not the right term to use. Anyway, I was wondering what causes this to happen. There must be something the screen reader is not picking up, or the screen reader cannot pull focus to the link or control.

Do you have a site we can look at? The first two things that pop in my head
are the tab order attribute, and incorrect handling in modal windows.
