I tested with JFW 9 and JFW10. The only issue that I noticed was that the graphic is unlabeled. It is also a little odd in that the prompts are read a second time after the form inputs. This was more of an issue with JFW10 with auto enter and exit of forms mode turned on. You might want to consider moving the print button to the end of the form in the tab order.
