Jan,

I tested with JFW 9 and JFW10. The only issue that I noticed was that the graphic is unlabeled. It is also a little odd in that the prompts are read a second time after the form inputs. This was more of an issue with JFW10 with auto enter and exit of forms mode turned on. You might want to consider moving the print button to the end of the form in the tab order.

Hmm, the only odd thing I noticed (using Jaws 10, Build 512) was that
the labels for the signature boxes at the bottom were all read together:

"Signature of StudentDateSignificant Other*Date"

The main form itself behaved quite well.

I tested using Voiceover with Leopard OS 10 and Safari. I had no luck
with it. I interacted with the scroll area, after listening to it and
there was no edit text field, no submit button. This could be a
Voiceover issue, but it would not have been accessible for me.

That is because those items are not really form fields. Reading line by line or word by word I found them understandable. The form appears to expect that those item will be completed with pen and paper, not uncommon for release forms. Which version of Adobe reader are you using. We found some significant problems with forms with Reader 8 and JAWS 9 and 10.

Hello Jan,

I tried to use this form with JAWS 7.0 and found the following issues:
1. Alternate text for an image was missing. In the absence of the alt, JAWS
read 'Graphic' living the users guessing what the image might be.
2. Headings were not marked up.
3. Labels for the form fields were assigned an incorrect reading order, I.e.
For the 'Name' fields, labels were read after the form fields
by JAWS. Same was the case with 'Address' fields.
4. Signature fields and Date fields were read as text assuming they have to
be filled up after taking printout .
5. Instruction "* required..." was placed at the bottom of the page which is
likely to go unnoticed by many of the users.
6. "Print" button was read as the first item in the tab order.

I understand what you mean. I should have clarified further. All
those labels appear run together on my braille display.

Doing a second check I found a couple other examples of this:


"FirstMiddleLast"
(This is read after the Name form fields.)

"Graphic NumberStreetCityZipcode"
(Before the address fields)

There is also the "Graphic 9" near the start of the page.

(So my experience matches Priti Rohra's findings pretty closely.

I'm using Acrobat Pro 9.1.1 (& Jaws 10).

I think that you may have found a previously unidentified (by us) feature of Adobe Live Cycle Designer. Apparently when multiple "text" boxes are used in a "row" within the design the resultant PDF does not have space characters between the elements. This seems to be resolved if you add the spaces to the text within the text boxes but we will need to do a bit more testing to be sure. We use a somewhat different design standard than the one the example provided by Jan, so we had not encountered this particular issue before.

We attempt to use the captions feature for all of the form fields when possible. In the design that was submitted we probably would have chosen to use the bottom caption element for each of the fields and made all of the "inputs" editable. But we may have more latitude to make design changes than Jan.
